In North Shields there are nine care homes:

Each of these Care Homes is linked with a single GP practice, and we encourage all residents to be registered with that practice. This helps residents, staff and the practice with good continuity and good relationships, and this improves the quality of the experience for residents.

Enhanced Health in Care Homes means a number of things are happening:

  • More ‘ward rounds’ which include more people like the physio or Occupational therapist or social worker
  • Regular reviews of ALL the care needs of the residents including reviews of medication and whether all the medications are needed
  • Checking out important issues like hydration, mental health and wellbeing, nutrition, falls prevention, wound care etc
  • Helping maximise the independence of each resident and avoid unnecessary hospital admission
  • Making sure we plan for end-of-life care and coordinate all the people who are helping to give care
  • Supporting and educating the workforce and making best use of technology

And there is a Palliative Care team who are also working into our Care Homes.

And we can call upon support from Occupational therapists, Physios, Geriatricians and others from Care Point and NHCFT and Local Authority (Council) to make sure that anyone in a care home gets exactly the same high quality care that someone who is housebound in the community would get.
